Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Amber Haywood – Care Coordinator
Hi, my name is Amber Haywood and I’m a care coordinator with the Care Alliance of Western Mass. As a care coordinator, I work with the Accountable Care Organization in your insurance to create goals, assess your needs, and work with you to get to a healthier place in your life. I also do things such as community resources, get connect with social service agencies for things like housing needs, medical supplies, in-home services, and things like that. I truly enjoy this job because we often meet people that are facing challenging times in their lives. And so to be a part of the team that helps them move from a challenging time to achieving their goals and achieving independence is really meaningful to me. The resilience and strength my members have is really inspiring and knowing that I help one person get to a better place makes this job very rewarding for me and why I truly love what I do.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
