We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Anabel Rivera – Care Coordinator
My name is Anabel Rivera and I’m a care coordinator for Care Alliance of Western Mass. As a care coordinator I work with the primary care doctor and the health insurance and making sure the member is safe and healthy at home. I connect them with resources in the community and also advocate for their well being. I like what I do and maybe because I have a background of taking care of family members that were not well taken care of and now I feel that I have to do the job and not face what I lived before, and I really like what I do.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
