Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Calista Solivan – Care Coordinator
Hi, my name is Calista Solivan I’m a care coordinator with Care Alliance of Western Mass. I’m a care coordinator that can help you with different areas in the community. There’s areas where I can help you with resources from the community. For example like DDS, MRC, and different areas if you’re in need of food. We could select different areas of the community where you could go to pantries. There’s other areas where we could help with medical equipment, advocate for you with your PCP. We help with different referrals from the doctors and we help you with the community as well. It’s very rewarding helping members that are in need with different disabilities. The best part of my job is that I don’t look at the disability, I just look at the part where we need to help these members get to where they need to and learn how to be independent and take the right route in their lives with their health.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
