At Care Alliance of Western Massachusetts, our Care Coordinators make a difference in healthcare consumers’ lives. We work closely with MassHealth ACO and MCO members with chronic or disabling conditions. Our team provides person-centered care coordination through in-person and phone communications. We learn what each member needs and wants and we connect them with the right community services, supports, and resources.

Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
