Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Hannah Black- Care Coordinator

Hi, my name is Hannah Black. I am a care coordinator with the Care Alliance of Western Mass. In this work we help members to maintain a healthy and happy life at home. We work directly with Mass Health or your insurance company to make sure that we are helping you to identify and realize your chosen goals and to obtain them. We work with community agencies to help you apply for other benefits such as food stamps, fuel assistance, or subsidized housing. We can also assist in setting up for transportation to medical appointments or connect you to other community services. Our main goal though is to always hope that we can keep you at home in the healthiest possible way. I love this job because I get to see the smile on everyone’s faces each and every day. I also hear words of gratitude and it makes my job so enjoyable.

Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
