Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Jeena Luna – Care Coordinator
Hi, my name is Jeena Luna and I am a care coordinator with the Care Alliance of Western Mass. My job as a care coordinator is to utilize and provide resources that assist our members in need to obtain self sufficiency ultimately creating a better way of life. We strive to help our members connect with providers and maintain relationships and open communication with their ACO. We also provide assistance with improving basic life skills and navigating community resources. We as a team work closely with the insurance companies and the case managers they have assigned to our members to facilitate communication and avoid lapses in insurance. I can assist with maintaining community relationships, finding programs to assist in the home for personal care, and helping members apply for and appeal for financial resources. We do this by creating and maintaining care plans that outline the members needs and conducting routine conversations and home visits to provide a higher standard of care. Our ultimate goal is to keep our members home and as independent as possible within their disability. I chose this field because I like being able to help people and I feel like everyone should have access to the necessary tools to better their quality of life.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
