Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Jeremy Vega – Care Coordinator
Hello, my name is Jeremy Vega. I am a care coordinator with the Care Alliance of Western Mass. As a care coordinator, my role is to assist you in completing your goals, outlining your care plan, and help you get resources to get through some of your challenges. I can also further assist you with overcoming your challenges by having access to ACO partners. These are care managers that work with your insurance companies to help us assist you with your lives. An example of an ACO helping us would be; if you need to get a referral for some kind of medical equipment and you can not get ahold of your ACP, you could then call me your care coordinator. I will try to get as much information as possible from you and due to being part of the ACO program, I could contact one of the care managers that works with your ACP and possibly get a referral done that way. Along with being able to assist you overcoming your challenges goals, I also enjoy knowing that the time and work I put into my members is put into good use serving others. As your care coordinator, I hope that we can foster great communication and report. That way I can serve you best in the times to come and during this new period in all of our lives.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
