Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Joel Torres – Care Coordinator
Hi, my name is Joel Torres, I am a care coordinator through the Care Alliance of Western Mass. I work closely with the ACOs and MCOs and care managers to provide long term supports and services to our members. As a care coordinator working with the ACOs and MCOs we design a person-centered care plan for that individual to best meet their goals. As a care coordinator, I am also responsible for giving individuals resources in the community. Such as to and from appointments, such as subsidized housing applications, food stamp applications, and fuel assistance applications. What I like about being a care coordinator is that I am able to assist members and ensuring that they’re getting the best compassionate and dignified care as possible. Thank you.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
