Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Mistie Ayala – Care Coordinator
Hi, my name is Mistie Ayala. I work with the Care Alliance of Western Massachusetts. I am a care coordinator here. As a care coordinator, I work with the insurance company and the case manager to help assist people within the community and to connect them with the resources within the community. Resources such as IEPs for schools and their children depending on the member and their age, we can help assist with housing, we can help assist with the department of developmental services, which is DDS, any and all areas the member may need. We try to make sure they’re stable, independent, and self-sufficient.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
