Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Nicole Rodriguez – Care Coordinator

Hi, my name is Nicole Rodriguez, I’m a care coordinator for Care Alliance of Western Mass.  I work with members between the age of 3 to 65 and part of my role is basically just to get the resources that our members need in order to help them to become successful out in the community. One of the things that I actually enjoy working as a care coordinator is being able to develop a care plan that actually works for the member. My job is actually just to listen to them, actually to develop something that works for them, and one of the greatest things that I like is to actually see them meet their goals in their community. Things they’re just not able to kind of see and just have that really good connection and be able to feel that you are able to connect and be able to accomplish these needs and to have a healthy life that you know that they never thought they were going to have and be able to have those accomplishments with the member I think is just rewarding. I just feel that this program helps accomplish that and be able to reach those goals and meet those needs.

 

Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
