Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Roxanne Crussana – Care Coordinator

Hi, my name is Roxanne Crussana. I am a care coordinator for the Care Alliance of Western Mass. As a care coordinator we ensure that people get connected to there health and social resources. We work closely with the ACO, which is the insurance company. By talking to each other at least one a month to ensure that either your barriers and or goals are being addressed and that we’re working together to get them stable, settled. As a care coordinator, we get you connected to resources within the community. Whether that be transportation to your medical appointments, fuel assistance, housing assistance, child care if need be, food stamps, or to the food bank/food pantries, and then for even some people helping them find jobs. As a care coordinator, I love that people know that I have the access and the ability of knowing health information, as well as community resources. I want them to trust me to be able to do my job and to help them get connected to these things, to make them more stable within their lives.

Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
