Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Yabet Ramos – Care Coordinator
Hi, my name is Yabet Ramos, I am a care coordinator for the Care Alliance. I assist members between the ages of five through sixty-four. We assist with coordinational services in the home as well as coordinational services in the community. Our goal is for members, clients, patients, we could call them in different ways to assist them to be able to have a safety and be able to live in their home as independently as possible. I enjoy what I do because I love assisting others. As a person who grew up with a handicapped parent who didn’t have the services as they have now. So I enjoy helping others to be able to have what they need in their home.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
