Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for eligible MassHealth ACO and MCO members ages 3-65.

Yolanda Rivas – Care Coordinator
Hi, my name is Yolanda Rivas, I’m a Care Coordinator here and Western Mass. My responsibility as a care coordinator is to work closely with the member’s ACOs and administration’s offices to connect our members within the community services with education, leisure, and social services. One of the things that make me happy working with my members here at the Care Alliance as a care coordinator is making sure my members have a smile at the end of the day knowing that they have been connected with the services that they need to live a good quality of life and remaining at home.
Care Alliance of Western Massachusetts works with residents of Hampden, Hampshire, Berkshire, and Franklin Counties. We also provide services to residents of Athol, Petersham, Phillipston, and Royalston in Worcester County.
